The scheme “State Level Awards for Divyang Persons” is implemented by the Social Welfare Department, Government of Uttarakhand, to honor the exceptional achievements and contributions of persons with disabilities in various fields. Under this scheme, a cash prize of ₹8,000, along with a medal and a certificate of appreciation, is awarded to outstanding individuals in categories such as disabled employees, outstanding sportspersons, and best self-employed service providers. The awards are distributed annually on the occasion of World Disability Day (December 3) to recognize and encourage excellence among persons with disabilities.

Applications are invited via public notice or advertisement in newspapers before December 3 each year.
Step 1: The interested applicant should visit (during office hours) the District Social Welfare Office and request a hard copy of the prescribed format of the application form from the staff exclusively entrusted to issue and collect filled-in applications.
Step 2: In the application form, fill in all the mandatory fields and attach copies of all the mandatory documents (self-attested, if required).
Step 3: Submit the duly filled and signed application form along with the documents to the District Social Welfare Officer.
Note: For this award, applications are invited in newspapers.
Post-application Process
Verification:
- District-level verification of documents and achievements is carried out.
Screening and Recommendation:
- Verified applications are forwarded with recommendations to the Director, Social Welfare.
- A screening committee at the state government level evaluates and selects eligible candidates in each category.
Award Distribution:
- The awards are distributed on the occasion of World Disability Day on 3rd December.
